Midwest Catholic Championships Registration

To register for the Midwest Catholic Championships, please complete the following 3 steps:

1. Click on "Athletic Contract" below. When the pdf file opens, print the contract and then close the pdf file window. Complete and sign the contract, and return a copy to:

2. Mail the appropriate Entry Fee to the above address.

$120 for Men or Women
$180 for both
(no separate fee for Reserve teams)

3. Fill in the information below and click "Submit Form". You will receive an email confirming your entry.

You will receive an e-mail in the fall, reminding you to submit your roster on this website.
